Requesting Military Records

Requesting Certified Copies of your DD-214 or Separation Papers:
Go to http://www.archives.gov/veterans/evetrecs/ , scroll down to the bottom of the page and follow the instructions there. The last part of the process will require you to print out a “Signature Verification Form”, sign it and mail or fax it to the National Personnel Records Center. If you fax it, you should get two copies of your DD-214 in about two weeks. Mailing it will probably add one or two weeks to the process.

This is an Adobe PDF fillable version of the SF-180. It will allow you to type the needed information into the form using your keyboard. You will still need to print, sign and mail the form. Depending on what service records you are requesting, you will need to follow the mailing instructions on the last page. The SF-180 allows you to request a DD-214, Separation Papers, Medical Records or your entire Official Military Personnel File (OMPF). Depending on your request, it can take somewhat longer than requesting your DD-214 via the Online Request process for your DD-214. There is usually no charge to a Veteran for copies of a DD-214 or Separation Papers. A service fee may be charged for other documents depending on the circumstances.
